NEW PATIENTS: *** Check your email for an Intake Form from me through Noterro*** If you don't see it, check your SPAM folder. We cannot begin an appointment until the forms are completed. You will be financially responsible for office time used to complete forms unless you have specific needs that I can accommodate. Insurance cannot be charged for this time. If you have barriers to filling out forms online, please email me and we'll work something out. Thank you. ***NOW ACCEPTING WORKER'S COMPENSATION CLAIMS!*** Please contact me and/or my biller prior to booking. We need to get your case information and put your info into the billing software before we can begin. FINANCIAL HARDSHIP NOTICE: If you truly need the work and truly can't afford the price, please reach out and let me know your situation. I keep a little time most weeks to accommodate sliding scale.
NOW ACCEPTING WORKER'S COMP
Auto accident and worker's comp claims:
Please schedule 90 minutes for the first visit and 60 minutes for subsequent visits. Even if you're already established in my practice, this applies because it's a new type of case.
Health insurance:
Standard appointments are 1 hour.
If you want a longer session, the remaining time can be paid out of pocket. Example: 90 minute visit when you have a $15 copay for an hour session. Expected cost would be $15 copay + the fee for 30 minutes session.
Please note that this is the expected cost and may not reflect actual copays, coinsurance, deductibles or other financial responsibilities, as reimbursement may change from time to time.
Out Of Network Health Insurance:
Book your session for however long you like but keep in mind that most insurance will not cover more than 1 hour. When you pay, I add insurance-approved codes to your invoice to create a superbill, which you can download from your Noterro profile and submit to insurance for reimbursement.
Self- Pay:
Choose the session length that serves your needs best.
